Text messaging program

With your consent, Courtney Funderburk, DNP, FNP-C may send text messages about scheduling, appointment reminders, and general office questions. This program is for administrative communication. It is not for marketing, medical advice, diagnosis, treatment, or emergencies.

Your consent and choices

You may opt in by checking the optional, unchecked SMS consent box on our contact form and submitting the form with your mobile number. Providing your phone number or requesting a callback alone does not give SMS consent. Consent is not a condition of receiving care. You may request a telephone callback without agreeing to texts.

Frequency and charges

Message frequency varies based on your requests and appointments. Message and data rates may apply according to your wireless plan.

Stop messages or get help

Reply STOP to opt out of SMS messages at any time. After opting out, you may receive a confirmation of your request. To request assistance, reply HELP, call 406-945-9045, or email courtney@funderburkdnp.com. Business hours are Monday–Friday, 9:00 AM–6:00 PM Mountain Time.

Protect your medical information

Do not send symptoms, diagnoses, medications, test results, insurance information, photographs, or other sensitive medical information by SMS or through the website form. Ordinary text messaging is not intended as a secure clinical channel. Use the Carepatron patient portal as directed by the practice, or call the office for clinical questions. Text messages and website requests are not monitored for emergencies. For emergencies, call 911. A callback request does not confirm an appointment. Call the office to schedule.

Delivery and your mobile number

Delivery may be delayed or unavailable because of carrier, network, or device issues. Carriers are not liable for delayed or undelivered messages. Please notify the practice if your mobile number changes, and provide only a number you are authorized to use.

Privacy

Read our Privacy Policy for information about collection, use, and processing of contact and SMS consent information.

No mobile opt-in or text message consent will be shared with third parties or affiliates. We do not sell, rent, or trade mobile numbers, SMS opt-in information, or text message consent. SMS opt-in information and consent are excluded from any sharing of information for marketing or promotional purposes.

We use service providers to operate our website, store form submissions, and deliver messages solely on our behalf. These providers may process information only as necessary to perform those services and are not authorized to use it for their own purposes or to transfer consent to another business or messaging program. We may disclose information when required by law.
